New Patients
Dear Parents:
Welcome to Polar Pediatrics, the medical practice of Dr. Monique Karaganis. We are welcoming new patients to our practice. I’m looking forward to meeting you.
As with any other relationships in our lives, we should make certain that we’re a good fit. Our first meeting will be a “getting to know you” opportunity to meet you and your child, and for you to meet me. Together we’ll review my policies and we’ll discuss your expectations for our relationship. At this initial meeting, we’ll collectively decide if we want to work together. If we conclude that you might be more comfortable with another pediatrician, I will provide you with a list of Anchorage pediatricians.
New Patients, please fill out and print the forms below. Bring in the completed forms on your first visit. Also remember to bring your child's immunization records and insurance information. You may want to call your insurance company and find out your co-pay, deductible and coverage for both sick and well visits.
