Dear Parents:

Welcome to Polar Pediatrics! We are happy to invite new patients to our practice. We’re looking forward to meeting you.

We understand that your relationship with your pediatrician is an important one and we want you to embrace and enjoy your choice. We also want you to have fun when you visit us. That’s why our first meeting is about getting to know each other; to make sure we are a good fit. It’s an opportunity for you and your child to meet Dr. Child and for us to meet you.

We’ll start by reviewing our office and practice policies together and then we’ll discuss your expectations for our relationship. Collectively, we’ll decide if we want to work together. You deserve a doctor that is a good fit for you.

How We Work

• We answer our own phones! If you get our voicemail, please leave just one message and feel confident that we will get back to you at the very earliest opportunity.
• We love to start your appointment right on time! Arriving late (or even too early) can cause disruptions for other patients. Please consider this when scheduling your appointment and communicate with us if unforeseen circumstances prevent you from being on time.
• So that we can spend time exclusively focused on you and your child, we’ve included links to our new patient forms below. It is required that you fill your forms out prior to your first visit, print them and bring them along.

Also remember to bring:

• Your child's immunization records
• Your insurance information
• Your ID

Before you come in, you may want to call your insurance company and find out your co-pay, deductible and coverage for both sick and well visits.
